At the Assin North Municipal Assembly, Antwi-Boasiako polled 18 out of the votes cast by the 47 assembly members with 29 rejecting him.
The votes, which were less than 50 per cent, means the President’s nominee was rejected outright.
Mr Antwi-Boasiako needed two thirds of members present to be confirmed Municipal Chief Executive.
Twenty-four votes representing a 50 per cent poll would have given him a second shot at the position after 10 days.
The assembly members were very quiet after the Electoral Officer, Kennedy Opoku, announced the results.
Mr Antwi Boasiako, looking disappointed, occasionally whispered something to the Regional Minister, Mr Samuel Sarpong, and his Deputy, Mrs Queenstar Sawyerr, both of whom looked disappointed.
Mr Sarpong had earlier urged the assembly members to re-elect the MCE to enable him to complete his good works.
He did not comment on the election and left the grounds almost immediately.
At Assin South, Madam Appiah-Kubi polled 17 votes representing 50 per cent of the 33 votes cast after appeals from her and the regional minister to the assembly members to confirm her.
She has 10 days within which she could be confirmed or rejected.
